1. Complete and submit the form below to let us know you're coming.
2. Attend a worship service between June 23rd and July 28th and complete our green new visitor form. 
3. Complete our online survey by July 31st. The link to the survey will be emailed to you after you have visited & filled out the new visitor form.
4. Receive your $20 Starbucks gift card!
